Meet the Primary Care Practice Team


Luz S. Ramos-Bonner, MD, FACP, AGSF, CMDNetwork Medical Director

Dr. Ramos-Bonner is a fellow of both the American College of Physicians and the American Geriatric Society. She previously was the medical director of the LIFE program in Trenton, NJ, and a primary care physician and geriatrician with LifeCare Physicians of Bordentown (NJ) and the Sister Hyacintha Program for the Homebound. She completed a fellowship in geriatric medicine at Albert Einstein Medical Center in Philadelphia and has served on the faculty of the Internal Medicine residency program at St Francis Medical Center.


Claudine H. Clarke, MD - NewCourtland LIFE Medical Director

Dr. Clarke has more than 20 years of primary care experience, along with a depth of knowledge serving as a geriatrician. She has served as attending physician in the geriatrics division at Einstein Medical Center, and staff physician at several nursing homes and assisted living facilities in the Philadelphia area.


Alan P. Berg PhD, MD 

For decades, Dr. Alan Berg has provided expert medical care for home-confined senior citizens in Philadelphia. For this, he was awarded ‘Practitioner of the Year’ from the Philadelphia County Medical Society. Before starting his second career in medicine, he was faculty member in Biochemistry and Physiology at the Medical College of Pennsylvania, and a researcher in the field of biologic aging.


Sivakoti N Guda MD, FACP, CMD

Dr. Guda completed a geriatrics fellowship at the Virginia Commonwealth University Health System in Richmond. He previously served as a faculty member of the internal medicine residency program at St. Francis Medical Center in Trenton, NJ, medical director of several nursing homes and as a geriatrician with Life Care Physicians of Lawrenceville, NJ. Dr. Guda has been practicing geriatrics for over 12 years; his areas of interest are patient-centered care and palliative care.


Vladimir Zagrebelsky, MD

Dr. Zagrabelsky has conducted research in allergy and immunology as a fellow at the Ukrainian Academy of Medical Science and has authored or coordinated a number of studies in this field as well as in cardiology and heart disease for medical research in both Russia and the U.S. He completed an internal medicine residency and a geriatrics fellowship at Einstein Medical Center, in Philadelphia, PA.


Mary Schuler MD

Dr. Mary Schuler has practiced in the Philadelphia region since graduating from Jefferson Medical College in 1995. She has extensive experience caring for geriatric patients in outpatient, hospital, rehabilitation, and residential settings. She is board certified in Internal Medicine and she is a member of the American College of Physicians and the American Medical Doctors Association.


Renee E. Connolly CRNP, DNP

Dr. Renee E. Connolly is a board certified nurse practitioner, specializing in geriatrics. Renee has decades of experience serving the elderly, in various clinical positions, within the Philadelphia region. Both her clinical work and passion is caring for the elderly. Additionally, Renee has mentored numerous nurse practitioner students from various schools of nursing in the area.
