NewCourtland LIFE Program
Modeled after the nationally recognized Program of All-Inclusive Care for the Elderly (PACE), NewCourtland LIFE provides an individualized and holistic approach to providing healthcare and social services to the seniors it serves.
Eligible participants must be:
- Aged 60 or older
- Certified by the state of Pennsylvania to be in need of nursing home care
- Able to live safely in the community at the time of enrollment with LIFE services
- Eligible for Medicare/Medicaid, or able to pay privately
- Residing in the following zip codes in Northwest Philadelphia:
- 19118, 19119, 19120, 19126, 19127, 19128, 19129, 19132, 19138, 19141, 19144 and 19150
