Medicaid Success at Home and Germantown Home Long-Term Care Partnership

  • Posted on Feb 23, 2022

Many family caregivers struggle with what documents are needed when placing a loved one in a nursing home. So in an effort to make sure you are prepared, Medicaid Success At Home and NewCourtland Germantown Home have formed a partnership to help guide you through the process.

Medicaid Success is a full-service Medicaid planning and application service.  They specialize in assisting people to become eligible for benefits and apply even in the most challenging situations. For families concerned about the cost of long-term care and spending down assets or losing income, they can provide Medicaid compliant strategies to preserve assets and income while becoming eligible for Medicaid. As part of the application process, they act as the authorized representative so that the patient does not have to deal with the county assistance office and will see the process all the way through to approved Medicaid eligibility, including a fair hearing if necessary. Patients will be supported and guided by experts every step of the process, helping them achieve Medicaid benefits as soon as possible. If you have any questions or would like to set up a pre-eligibility screening/consultation at no cost, contact Lynn Donato, CMP™ at 973-898-5959 or email lynn@seniorresourceplanning.com.

NewCourtland Germantown Home is situated on 13.5 acres in the heart of historic Germantown and is part of NewCourtland’s flagship campus. The Home offers a variety of social, educational, spiritual, and therapeutic programming in order to create a warm, welcoming environment. Staff is available 24-hours a day to answer all questions and concerns and will communicate with you regarding the care of your loved one.  If you are looking for more information about long-term care or memory support services and would be interested in taking a tour of the Home, please contact Alison Corter, Director of Business Development at 610-662-2970 or email Acorter1@newcourtland.org.
