Nazareth Hospital and NewCourtland Senior Services Team Up to Meet the Needs of Seniors in Northeast Philadelphia

Nazareth Hospital and NewCourtland Senior Services have partnered to meet the needs of seniors in our community. For years, Nazareth has focused on the healthcare needs of older adults with its Balance program, Senior Care Emergency Department Nurse Navigator and community outreach. As part of this new arrangement, NewCourtland will help Nazareth strengthen this outreach by co-presenting aging-related community education seminars. In addition, the hospital will also provide acute care for participants in the NewCourtland LIFE Program when needed.

Based in Philadelphia, NewCourtland is a non-profit provider of affordable housing, healthcare, and supportive services that serves thousands of older Philadelphians each year. NewCourtland. LIFE is a comprehensive program that helps low-income adults 55 and older stay in their homes as long as possible by providing high-quality healthcare and supportive services – a welcome alternative to long-term care facilities for seniors and their families. The LIFE team provides eligible seniors with primary and specialist care; physical, occupational and speech therapy; prescriptions; home modifications if needed; health insurance; transportation; meals; and adult day care.

Nazareth Hospital and NewCourtland’s first project as partners was a Dementia Workshop, held on December 19 at the NewCourtland LIFE Center at Roosevelt Plaza. The workshop educated caregivers on dementia care, providing information on the basic functions of the brain, differences between dementia and Alzheimer’s disease, types of dementia and more.
