From our housing to our dedicated Primary Care Practice to our comprehensive on-site programs, NewCourtland offers a unique combination of services to help seniors maintain the independent lifestyle they cherish.
The Primary Care Practice is a dedicated team of experienced gerontologists, nurse practitioners and home-visiting doctors dedicated to serving seniors.
The Services On-site Program provides Service Coordinators to assist with everything from insurance forms to diet advice to arranging transportation to senior centers.
NewCourtland offers qualified candidates the opportunity to transition out of the nursing home and back to the community to help seniors live as independently as possible for as long as possible.
In October of 2019, NewCourtland entered into a partnership with United HealthCare to provide supportive housing to United HealthCare members who receive Medicaid and are homeless and chronically ill. It is our shared belief that by providing housing with supportive services, these individuals can care for themselves and significantly reduce the number of emergency room visits.
