From our dedicated Primary Care Practice, to our comprehensive on-site programs, to our nursing home, NewCourtland offers a unique combination of services to help maintain an independent lifestyle and also provide for those who may need more care.
The Primary Care Practice is a dedicated team of experienced gerontologists, nurse practitioners and home-visiting doctors dedicated to serving seniors.
The Services On-site Program provides Service Coordinators to assist with everything from insurance forms to diet advice to arranging transportation to senior centers.
In October of 2019, NewCourtland entered into a partnership with United HealthCare to provide supportive housing to United HealthCare members who receive Medicaid and are homeless and chronically ill. It is our shared belief that by providing housing with supportive services, these individuals can care for themselves and significantly reduce the number of emergency room visits.
