Our Services On-Site Program Service Coordinators are equipped to provide connections to community resources for health care, pre and post-hospital needs, socialization, nutritional support and mental health counseling.

In addition to social services, our Services On-Site Program offers a variety of ancillary services including: regular health promotion education and screening programs, home support services to assist with housekeeping tasks and access to our senior centers and various other supportive services.
